Community Health Connection

Community Health Connection (CHC) is a federally qualified health center dedicated to delivering accessible, high-quality care for individuals and families across northeast Oklahoma. The new Tulsa facility reflects this mission through a welcoming design that emphasizes transparency, trust, and patient comfort. Expansive glazing, a light-filled lobby, and thoughtfully selected finishes create an environment that supports dignity, well-being, and long-term patient-provider relationships.

The facility was designed to enhance both care quality and operational efficiency.
