' A true medical home'

Community Health Connection (CHC) is a federally qualified health center (FQHC) and Patient-Centered Medical Home (PCMH) dedicated to providing high-quality, accessible, and culturally responsive care for individuals and families in northeast Oklahoma. The design of the new Tulsa facility reflects this mission, embodying both transparency and trust through clean architectural forms and a welcoming entry sequence. Expansive glazing and a light-filled lobby create an atmosphere of openness and dignity, while carefully selected interior finishes establish a calming, professional environment that supports patient comfort. These design elements are purposefully aligned with CHC’s vision of being a true “medical home”—a place where long-term relationships between patients and providers are fostered in a supportive and respectful setting.

Equally important, the facility’s layout was planned to elevate both the quality and efficiency of care delivery. Clinical services—including family medicine, pediatrics, dental, behavioral health, and pharmacy—are strategically connected to streamline operations and enhance coordination across disciplines. Wayfinding is intuitive, ensuring patients can easily navigate the building while maintaining privacy and security. Flexible planning anticipates future growth, allowing the facility to adapt to evolving community needs such as expanded pediatric care and bilingual health education programs. By integrating thoughtful architecture with operational strategy, the new Community Health Connection facility will strengthen CHC’s ability to deliver comprehensive, compassionate care while advancing its mission to improve the health and well-being of the communities it serves.

The Hardesty Leadership Center has become not only a home for the Girl Scouts of Eastern Oklahoma, but also a civic beacon—hosting events, workshops, and community gatherings that amplify the organization’s role in shaping future leaders.
